Pigmentation and uneven tone

Brown patches, sun spots and a tone that will not even out. The most important thing here is working out which kind you have, because one kind gets worse when you treat it with a laser.

What it actually is

Sun damage produces defined brown spots with clear edges, usually on the areas that get the most light. Melasma produces soft, symmetrical patches, often across the cheeks, forehead or upper lip, and it is driven by hormones as well as light. Post inflammatory pigment is a flat mark left where the skin was previously injured or inflamed. They look similar in a mirror and behave completely differently under treatment.

What causes it

  • Ultraviolet and visible light, including the light coming through a window and off a screen.
  • Hormonal change, particularly pregnancy and the combined pill, which is the usual trigger for melasma.
  • Heat, which is an independent trigger for melasma and the reason some clinic treatments make it worse.
  • Any previous inflammation, from acne to an ingrown hair to a badly chosen peel.

Before you book anything

A single pigmented lesion that is changing, asymmetric, has an irregular border or more than one colour needs a doctor rather than a medispa. Take it to your GP. We will not treat any lesion we have not been able to satisfy ourselves about.

Nothing on this page is a diagnosis. It is what we would say in the room, written down so you can read it before deciding whether to come in.

What treatment can do

Defined sun spots respond quickly and often clear in one or two sessions. Melasma can be controlled and made much less obvious with a combination of gentle resurfacing, pigment inhibiting homecare and daily protection. Post inflammatory pigment usually fades on its own, and we can shorten that considerably.

What it cannot do

Melasma is not curable and it relapses. Any clinic promising to clear it permanently is either misunderstanding it or hoping you will not come back to ask. Aggressive pigment lasers on melasma frequently rebound worse than the starting point, and we will decline that treatment rather than take the booking.

What a course usually looks like

Sun spots: one to three sessions, a month apart. Melasma: a twelve week control programme built mostly around homecare, with light in clinic treatment on top, then maintenance for as long as you want it held. Daily broad spectrum protection is not optional in either plan and there is no point starting without it.

Course pricing is lower than the same sessions bought one at a time, and spreading the cost is available over three months on anything above £300.

Where to start

Bring a photograph of it at its worst.

Most concerns look different in clinic lighting at eleven in the morning than they do at home at night. A photograph on your phone is genuinely useful and we would rather see one than take your word for it.
