Sagging skin and loss of firmness

A jawline that has stopped being a line. This is the concern most often treated with the wrong device, because four separate things produce the same look in the mirror.

What it actually is

The lower face changes because of four layers moving at different rates. Skin loses elasticity, the fibrous support net beneath it loosens, deep fat pads slide downward, and the bone underneath quietly recedes. What you see at the jawline is the sum of all four. Which one is dominant decides the treatment, and it is not something you can judge by looking.

What causes it

  • Elastin, which the body essentially stops replacing in adulthood.
  • Loosening of the support layer under the skin, which is what most lifting devices are actually aiming at.
  • Descent and deflation of the fat pads in the mid face, which empties the cheek and loads the jawline.
  • Weight loss, which very often unmasks laxity that was being held up from underneath.

Before you book anything

If the concern is a heavy neck with a distinct band running down it, that is platysma, and the honest answer is often surgical. Ask a facial plastic surgeon before you spend money here.

Nothing on this page is a diagnosis. It is what we would say in the room, written down so you can read it before deciding whether to come in.

What treatment can do

Tighten the support layer with focused ultrasound or radiofrequency, remodel the skin itself, or lift mechanically with threads. Expect a definition change that other people register without being able to name. Somewhere between two and four millimetres of lift is a realistic and genuinely visible result.

What it cannot do

None of this is a facelift and none of it is close. If there is loose skin you can take hold of, energy devices will disappoint you, and we would rather say that at the consultation than after you have paid for a course. A surgical opinion costs you nothing and we will tell you who to ask.

What a course usually looks like

Ultrasound and radiofrequency work as one treatment with the result appearing over three to six months, then repeated annually. Radiofrequency microneedling runs as three sessions a month apart. Threads are a single appointment with a result you see immediately and which then settles over three weeks and lasts twelve to eighteen months.

Course pricing is lower than the same sessions bought one at a time, and spreading the cost is available over three months on anything above £300.

Where to start

Bring a photograph of it at its worst.

Most concerns look different in clinic lighting at eleven in the morning than they do at home at night. A photograph on your phone is genuinely useful and we would rather see one than take your word for it.
