What it actually is

Two different things get called acne scarring. Flat brown or red marks are post inflammatory pigmentation and erythema, and they fade on their own over six to eighteen months. True scars are changes in the shape of the skin: shallow rolling dips, sharper boxcar edges, or narrow ice pick holes. Only the second group needs remodelling work.

What causes it

  • Inflammation that went deep enough to damage the collagen scaffold under the follicle.
  • Lesions that were squeezed or picked, which spreads the inflammation sideways.
  • Acne that was left untreated for years rather than months, which is the single biggest predictor.
  • Skin that scars readily anywhere, which you usually already know about from other injuries.

Before you book anything

If you still get active spots, treat those first. If the scars are raised and thickened rather than sunken, they are keloid or hypertrophic scars, and those want a dermatology opinion because injected steroid is usually the first move.

What treatment can do

Rolling and boxcar scars respond to controlled injury that makes the skin rebuild the collagen underneath. Expect a meaningful improvement rather than a flat surface. A realistic target for a full course is somewhere between forty and seventy per cent better, judged in the same lighting rather than from memory.

What it cannot do

Ice pick scars do not fill from above. They need a punch technique or excision, which is a minor surgical procedure and not something we do here. We will say so at consultation rather than sell you a course of microneedling that was never going to reach them.

What a course usually looks like

Three to six sessions, four to six weeks apart, then a wait. The collagen response runs for about six months after the last session, so the honest review point is six months out, not the week after. Most people need one top up course a year or two later.

Course pricing is lower than the same sessions bought one at a time, and spreading the cost is available over three months on anything above £300.
