Laxity, and the four things that cause it

Almost everyone who books a consultation about a softening jawline arrives having already read about four or five treatments, and having found each one described as the best. That is not much help. What follows is roughly how the decision actually gets made in the room.

Start with the layer, not the machine

Skin is not one thing. There is the surface, where texture and tone live. There is the dermis underneath it, which holds collagen and elastin and determines how firm the skin feels. Below that sits fat, arranged in compartments, and below that a fibrous support layer that a surgeon lifts during a facelift. Laxity can come from any of those, and usually comes from several at once.

So the first question at consultation is not which device you want. It is which layer is doing the most to produce the thing you dislike. That is why we scan and photograph before recommending anything.

What each option actually reaches

  • Focused ultrasound reaches deepest, including the support layer, so it does the most lifting from a single visit and needs the longest wait to judge.
  • Radiofrequency heats a broad, shallower area and needs a course. It is the best of the group for general skin quality and comfort.
  • Radiofrequency microneedling puts heat at a chosen depth through the skin, which makes it the strongest option where texture and laxity occur together.
  • Threads do not rely on heat at all. They reposition tissue mechanically, which is the right answer when something has descended rather than merely loosened.

The uncomfortable part of the conversation

There is a point past which none of these do enough. Where there is a lot of loose skin, an energy device will produce a change you can measure and cannot see, and a course of three will cost more than it returns. We would rather say that at the first appointment than at the third.

The other honest limit is time. Every treatment on that list works by prompting your body to make collagen, and that takes eight to twelve weeks whatever the brochure implies. If you have an event in a fortnight, the right answer is a facial and good sleep.
